Welcome to Formula 40 Plus! Formula 40 Plus takes the guesswork out of your renewal application process! You get 40 CEUs of training - we will consult with you to design a course schedule that will ensure you get all of your renewal requirements fulfilled. We keep track of your course certificates. We collect from you your completed renewal application, your check made payable to the Department of Social Services, and a copy of your Nursing Home Administrator license (if applicable). We review your application package for completeness and correctness and submit everything via certified mail to CCLD on your behalf.
