Getting Started

Welcome to Formula 40 Plus! Formula 40 Plus takes the guesswork out of your renewal application process! You get 40 CEUs of training - we will consult with you to design a course schedule that will ensure you get all of your renewal requirements fulfilled. We keep track of your course certificates. We collect from you your completed renewal application, your check made payable to the Department of Social Services, and a copy of your Nursing Home Administrator license (if applicable). We review your application package for completeness and correctness and submit everything via certified mail to CCLD on your behalf. Please note: CCL will accept no more than 10 CEUs completed in a single day! Certification renewal requires 40 hours; at least 20 of which must be conducted in a live course format, and 20 may be completed in a self-paced course format. *Group Home and STRTP users: Please note that not all of our courses are approved for GH/STRTP. Be sure to check whether you will receive credit before beginning a course.   Formula 40 Plus is valid for 1 year from the date of purchase or once 40 CEUs are complete - whichever occurs first. Activities of Daily Living (3 CEUs) Approved for: Adult Residential Facility (34024) Residential Care Facility for Elderly (34023) Group Home (34022) STRTP (34021) Board of Registered Nurses Activities of daily living (ADLs) are the common, everyday, learned activities that people perform to maintain their health and wellness. A resident in assisted living will probably require some level of assistance with their ADLs. This course provides step-by-step guidelines as well as tips and tricks for helping residents get through their daily care routines. Read more

Autism Spectrum Disorder (2 CEUs) Approved for: Adult Residential Facility (36569) Residential Care Facility for Elderly (36572) Group Home (36571) STRTP (36570) Approved by the Board of Registered Nurses This course provides students with an introduction to Autism Spectrum Disorder (ASD) and its impact on individuals and their families. Students will gain knowledge about the characteristics, causes, and diagnosis of ASD, and explore different types of ASD and their symptoms. The course also covers the impact of ASD on individuals, the importance of early identification, societal implications, attitudes towards individuals with ASD, and tools to support them in various settings.  Read more

Behavior Issues (1 CEU) Approved for: Adult Residential Facility (36418) Group Home (36419) STRTP (36420) Accepted by the Board of Registered Nurses By approaching behavior as communication, we are able to listen to what a resident may be trying to tell us, discover the reasons behind their behavior, and take steps to meet the needs or concerns behind it. This course also instructs the user on how to use a Functional Behavior Assessment and a Positive Behavior Approach. These concepts can be applied to address problem behaviors in residential care.  Read more

Bipolar Disorder (3 CEUs) Approved for: Adult Residential Facility (34083) Residential Care for Facility Elderly (34082) Group Home (34081) STRTP (34080) Board of Registered Nurses This course explains the basics of bipolar disorder including symptoms, diagnosis, and treatment. It also discusses how the disorder affects the lives of individuals with bipolar disorder and some controversial aspects. Read more

Common Health Issues (2 CEUs) Approved for: Adult Residential Facility (35916) Residential Care Facility for Elderly (35317) Board of Registered Nurses As we age, our bodies begin to work differently. Many bodily functions and systems begin to deteriorate, leaving a person with diminished capacity and vulnerable to many health issues. This module describes some of the more common health problems you'll encounter in a residential care facility, along with ways to help your residents cope with them. Topics include dehydration, urinary tract infections, constipation, aspiration, pneumonia, dental problems, and skin issues.  Read more
