Welcome to Formula 40 Plus! Formula 40 Plus takes the guesswork out of your renewal application process! You get 40 CEUs of training - we will consult with you to design a course schedule that will ensure you get all of your renewal requirements fulfilled. We keep track of your course certificates. We collect from you your completed renewal application, your check made payable to the Department of Social Services, and a copy of your Nursing Home Administrator license (if applicable). We review your application package for completeness and correctness and submit everything via certified mail to CCLD on your behalf. Please note: CCL will accept no more than 10 CEUs completed in a single day! Certification renewal requires 40 hours; at least 20 of which must be conducted in a live course format, and 20 may be completed in a self-paced course format. *Group Home and STRTP users: Please note that not all of our courses are approved for GH/STRTP. Be sure to check whether you will receive credit before beginning a course. Plus packages finished within 14 days of certificate expiration do not allow enough processing time and may risk a $300 CCLD late fee. The course is based on the premise that staff armed with the tools to understand what behavior is communicating can go a long way to improving the quality of care, and satisfaction of residents, families, and staff. Regulations: A Visit from Your LPA (2 CEUs) Approved for: Adult Residential Facilities (ARF) Residential Care Facilities for the Elderly (RCFE) Group Home (GH) Short Term Residential Therapeutic Program (STRTP) This two‐hour course outlines how to be prepared for a visit from your Licensing Program Analyst. Topics include frequency and potential reasons for a visit, what tools the analyst will use, accommodations you are required to provide for the inspection, and potential outcomes. Also discussed will be what constitutes a serious deficiency, the most commonly cited deficiencies, and citations. When, how, and if an appeal is appropriate will be discussed. Dementia: Beyond Disease (6 CEUs) Approved for: Adult Residential Facilities (ARF) Residential Care Facilities for the Elderly (RCFE) No GH/STRTP Board of Registered Nurses (BRN) National Council of Certified Dementia Practitioners (NCCDP) This six-hour course features the works of G. Allen Power MD, who is a board-certified internist and geriatrician. The course is centered around Dr. Power's first book, Dementia Beyond Drugs, which was named Book of the Year in 2010 by the American Journal of Nursing in Psychiatric/Mental Health Nursing. Additionally, Dr. Power's second book offers an experiential perspective on dementia rather than a medical one. Restricted and Prohibited Conditions and Postural Supports (2 CEUs) Approved for: Adult Residential Facilities (ARF) Residential Care Facilities for the Elderly (RCFE) No GH/STRTP Board of Registered Nurses (BRN) This two‐hour course identifies conditions that are either restricted or prohibited in residential care, as well as appropriate conditions for the use of postural supports.
